Miller/Davis provides a wide array of forms for use during Workers' Compensation claims in Michigan, as well as annual forms related to the claim. Many of these forms are used by an employer, an employee, or the employee's family in cases where the employee was injured, disabled, or killed while working his or her job. Other forms are used by the attending physician or the insurance agency.
